Meet the team

Nathalie Gilder – Director, Registered Manager & Nominated Individual

Nathalie is our Registered manager and nominated individual with the Care Quality Commission, her role is to ensure we are providing an outstanding service, and that we are complying with regulations.

Nathalie is also a trained Dementia Care Coach

Nathalie holds a Level 4 NVQ in Leadership and Management for Care Services, and has completed many Management specialist training courses such as Risk Assessment, Advanced Care Planning, Critically Reflective Supervision, and CQC Workshops.

Works Monday, Tuesday, Wednesday and Friday 09:00-17:00

Jessica Wilkins – Deputy Manager

Jessica role is to work for and assist our Registered Manager to ensure the smooth running of our service

Jessica has completed her QCF Level 3 in Health and Social Care and is working towards her QCF Level 5 in Health and Social Care (Managers Award), she also holds a Level 2 qualification in Team Leading and has completed many specialist training such as ‘Risk Assessor, Care Planning, and Engagement, assertiveness and challenging behaviors.

Isabel Osborne – Care Manager

Isabel’s role is to coach all new staff as well as carrying out spot checks and supervise all staff to ensure best practice, they work to the highest standards and care needs are being met. Isabel is also our medications manager and delivers training to all our team.

Holds QCF Level 5 in Health and Social Care (Managers award), OCF Level 3 in Health and Social Care, and provides training to our staff team in Dementia and Medication.

Della Swallow – HR Manager

Della’s role is to recruit new team members, coach and mentor them through the induction process, book all training (Mandatory, Specialist and Refresher) for all the team as well as ensure we comply with all legislation. Della is also a qualified trainer in Safeguarding and Manual handling and delivers training to all our team.

Holds a QCF Level 3 in Lead Adult Care and provides training to our staff team in Moving and Positioning, Understanding End of Life Care and is our Safeguarding Lead and Trainer. Della has also completed many specialist management training courses such as Assessing Competence, Crisis management, and Creative Thinking and Innovation.

Samantha Notley – Care Coordinator

Sam works FULL time in the office. Sam’s role is to coordinator all care, ensuring all visits are allocated to a regular team of carers. Sam is also responsible for carrying out all initial care and support assessments.

Holds an NVQ level 3 and 4 in Health and Social Care, and has completed many specialist training courses such as Care Coordinators Responsibilities, Good Governance and Auditing as well as being trained to deliver Moving and positioning training to our staff team.

Karolina Hryszko – Care Planning Coordinator

Karolina works FULL time covering both office and care visits. Karolina role is to review and update all care plans on a 3-6 monthly basis or following a discharge from hospital or change in care package. Karolina also devises Manual Handling Picture Plans.

Completed her QCF level 3 in Lead Adult Care in 2021, and Level 2 in Principles of customer services. Karolina has also completed many specialist management training courses, such as Risk Assessment, Conflict Resolution, Person Centred Care.

Sarah Francis – Team Leader

Works FULL time covering both office and care visits. Sarah’s role is to lead our team, carry out their spot checks, supervision, and ensure they work to the highest standards. Sarah also organises events and outings for both our service users and staff.

Working her way through her QCF Level 3 in Lead Adult Care, and completed her Level 2 in Health and social care in 2017. Sarah has also completed many specialist management training courses, such as Coaching, Mentoring and Challenging Conversations, Supervision Skills and Performance Management.

Liz Lombaard – Receptionist and Care Administrator

Works FULL time covering both office and care visits. Liz’s role is to support our HR Manager with Recruitment, Training, auditing and administration functions for our organisation. Liz is based in reception so will answer all calls and greet all visitors to the office.

Completed her QCF Level 3 in Lead Adult Care and has also completed many specialist training courses, such as Handling Information – GDPR and Data Protection, recording and reporting and Recruitment Masterclass.

Day off: Every Wednesday

Natasha Goldsborough Martin – Medication and Health Support

Natasha role is to audit, monitor and order Service Users medication, as well as dealing with any medication discrepancies. Actioning health concerns and making necessary health referrals.

Joanne Brown – Mental Health Champion

Joanne’s role as MH Champion is to raise awareness of mental health. Joanne is trained in and will be a source of support to all our staff and service users who maybe experiencing mental health problems and signpost them to relevant support and help, where appropriate.

Kelly Kennedy Smith – ‘Memories are Golden Community Hub’ Manager

Kelly’s role is to manage the Memories are Golden Community Hub day service. Kelly also organizes numerous fundraising events and activities to raise much-needed funds for the hub. Kelly also organizes numerous fundraising events and activities.
