Meet the team
Nathalie Gilder – Director, Registered Manager & Nominated Individual
Nathalie is our Registered manager and nominated individual with the Care Quality Commission, her role is to ensure we are providing an outstanding service, and that we are complying with regulations.
Nathalie is also a trained Dementia Care Coach
Nathalie holds a Level 4 NVQ in Leadership and Management for Care Services, and has completed many Management specialist training courses such as Risk Assessment, Advanced Care Planning, Critically Reflective Supervision, and CQC Workshops.
Jessica Wilkins – Deputy Manager
Jessica role is to work for and assist our Registered Manager to ensure the smooth running of our service
Jessica has completed her QCF Level 3 in Health and Social Care and will be working towards her QCF Level 5 in Health and Social Care (Managers Award), she also holds a Level 2 qualification in Team Leading and has completed many specialist training such as ‘Risk Assessor, Care Planning, and Engagement, assertiveness and challenging behaviors.
Isabel Osborne – Head of Care and Dementia Care Coach
Isabel’s role is to coach all new staff and ensure they work to the highest standards as well as spot-checking and supervising all staff to ensure best practice and care needs are being met. Isabel is also our medications manager and delivers training to every member of our team.
Isabel is also our Dementia Care Coach and is trained to provide ‘Step inside Dementia’ Training
Isabel holds a QCF Level 2 and 3 in Health and Social Care, Level 2 in Team Leader and has attended many specialist training courses
Della Swallow – Senior Recruitment and Training Coordinator
Della’s role is to recruit new team members, coach and mentor them through the induction process, book all training (Mandatory, Specialist and Refresher) for all of our team. Della also completes practice competencies to ensure all staff are confident and competent in their roles.
Lauren Roffe – Care Coordinator
Lauren’s role is to coordinate your care as well as carry out care plan reviews that ensure we are up to date and meeting your needs. She works full time covering both office and care visits.
Karolina Hryszko – Care Planning Coordinator
Karolina works full time covering both office and care visits. Karolina’s role is to review and update all care plans on a 3-6 monthly basis or following a discharge from the hospital or a change in the care package. Karolina also devises manual handling picture plans.
Karolina is working towards her QCF Level 3 in Health and Social Care
Sarah Francis – Team Leader
Sarah’s role is to lead our team, carrying out their spot checks, supervisions, and ensure they work to the highest standards. Sarah also organises events and outings for both our service users and staff.
Sarah is working towards her QCF Level 3 in Health and Social Care, and already holds a NVQ Level 2 in Health and Social Care; Sarah has completed specialist training courses in Care Planning, Partnership working to Safeguard Adults, Outcome based support
Kelly Kennedy Smith – ‘Memories are Golden Community Hub’ Manager
Kelly role is to organize events and activities for our staff, Service Users, and their families to enjoy as well as raising awareness of our service. Kelly is also the driving force behind the ‘Memories are Golden Community Hub’, which is a Not-for-Profit organisation devised by our team to bring together individuals living with Dementia and those over the age of 65 years who may be at risk of social isolation.
Kelly also organizes numerous fundraising events and activities.

Julie Jones – Quality Assurance and Well-Being Champion
Julie’s role is to visit all service users to ensure they are happy with the service being provided and to facilitate feedback and changes required within our service. Julie is also our team mental health and well-being champion.
Diane Gilder – Administrator and Reception
Diane role is to provide secretarial and administrative support to the team in order that they can provide an efficient service to all customers.
